Dear Parents & Guardians:
All students entering Kindergarten or any new registrant for September 2025 must have all the immunizations required for that grade, per the New York State Public Health Law, Section 2164. The schedule can be found here: NYS Health
Immunizations and Vaccinations Required
- All students entering Kindergarten or any new registrant for September 2025: must have all the immunizations required for that grade. Please submit to: llouischarles@nanuetsd.org or fax to: 845-624-0751.
- All students entering 6th grade & 11 years old: must receive a T-dap booster. Please submit to: gconklin@nanuetsd.org or fax to: 845-624-0751.
- All students entering 7th grade: must submit proof of receiving their 1st dose of the meningococcal vaccine before school starts. Please submit to: gconklin@nanuetsd.org or fax 845-624-0751.
- All students entering 12th grade: must submit proof of receiving their 2nd dose of the meningococcal vaccine before school starts. Please submit to: mhanigan@nanuetsd.org or fax to 845-624-0751.

If you need assistance with the cost of immunizations, please call the Rockland County Health Department to schedule an appointment: 845-364-2497.
Physicals:
Physicals are required under this public health law for the following grades:
- Kindergarten, 1st, 3rd, 5th, 7th, 9th & 11th
Please email or send the physical to your child’s school nurse. All links for the physical form can be found under “nurse” on the school’s website. The physical form can be emailed to the school nurse or faxed to: 845-624-0751.

Title 1 Schools Information
The schools in the Nanuet Union Free School District that are eligible to receive Title I federal funding for the upcoming year are: Highview Elementary School and A. MacArthur Barr Middle School. In accordance with the parent involvement goals of Title I of the federal No Child Left Behind Act of 2001 (NCLB), the Board of Education encourages the participation of parents of students eligible for Title I services in all aspects of their child's education, including the development and implementation of district programs, activities, and procedures designed to carry out NCLB parent involvement goals (refer to District Policy section 1900).
In addition, the school district is required to inform parents of their child’s academic progress on each of their state assessments. Please be assured that if your child takes a state assessment, you will receive notification by mail of your son’s/daughter’s results along with an overview and explanation.
