Dear Parents & Guardians:

I hope you & your family are enjoying the start of summer! Below please find a list of reminders that would be required for the 2026/27 school year.

• All students entering Kindergarten or any new registrant for September 2026: must have all the immunizations required for that grade. Please submit: llouischarles@nanuetsd.org or fax to: 845-624-0751.
• All students entering 6^th grade & 11 years old: must receive a T-dap booster. Please submit: gconklin@nanuetsd.org or fax to: 845-624-0751.
• All students entering 7^th grade: must submit proof of receiving their 1^st dose of the meningococcal vaccine before school starts. Please submit: gconklin@nanuetsd.org or fax 845-624-0751.
• All students entering 12^th grade: must submit proof of receiving their 2^nddose of the meningococcal vaccine before school starts. Please submit: mhanigan@nanuetsd.org or fax to 845-624-0751.

NYS Immunization Schedule:

If you do not have Health Insurance please call Rockland County Health Department to schedule an appointment: 845-364-2497.

Physicals: Physicals are required under this public health law for the following grades:

• Kindergarten, 1^st, 3^rd, 5^th, 7^th, 9^th & 11^th

Please email or send the physical to your child’s school Nurse. All links for the physical form can be found under “nurse” on the school’s website. The physical form can be mailed/emailed to your child’s school nurse or fax to: 845-624-0751.
